Can someone please tell me what a "4x5 type 5" holder is?
What 4x5 back will it fit? (graphic, graflex, graphloc?)

A Type 5 holder is the most modern design of graphic holder. With rare exceptions, it will fit all graphic and graphlock backs, and all modern cameras.
the Type 5 differs from the Type 4 at the top of the holder. A Type 4 has one light trap groove right above the film aperture while the Type 5 has a second that is intended to wrap around the body of the camera to further keep the dark in. All rubber (Graphic Riteways) and plastic holders are Type 5. Some of the wood holders with metal faces are type 5. Most of the wood holders with a wood light trap are type 4. Type 5 holders are longer than Type 4 but will fit the same camera (see photo, )
Type 5 holders also have 5 dots or holes in the dark slide to represent unexposed side. Earlier holders usually used 3. 5 dot holders are supposed to be light tight to IR wavelengths while 3 dots aren't.
